Taking Health Care Home (THCH) is the Corporation for Supportive Housing’s (CSH) national effort to prevent and end homelessness for people with chronic health problems. It's aimed at changing the way that housing, health and social support services are financed, organized and delivered. CSH works with selected states and localities to demonstrate how they can create supportive housing that ends homelessness for people with chronic illnesses. In Spokane, THCH is managed by a partnership, which includes the City of Spokane and Spokane County and the Spokane Housing Authority. Common Ground provided consulting services to these organizations and Northeast Washington Housing Solutions, a consortium of housing and service providers in Spokane County to produce this study, which detailed supportive housing models and ways to increase availability of permanent housing for people with chronic health conditions.
